That is one potential exception, yes.

Yes. A reposition allows a model to advance and an advance is one of the movement types that allow you to use an ingress point.

Although Puff of Smoke is still occurring in Step 12 of the attack sequence, it would occur after the model has had damage placed on it, been KO'd and then removed from the battlefield.

Not from damage caused by the Strained condition resolving, no. Strained is a non-player effect once its been applied to a character and thus no character, Unit or player is considered to have caused that damage.

Not quite. If some Blocked Terrain was between them that had a higher elevation than both characters, Line of Sight would be blocked.

Your looking for language that explicitly references a conditional statement like that found on Fierce Protector from Ahsoka Tano, Jedi No More. The second sentence starts with "Then, the character that moved..." which is conditional on the character having actually moved.

This is covered in the FAQ, which states the most recently placed Freedom Force token takes precedent when determining who is securing the objective.

There is no exception provided for a Wounded unit. An engaged Unit cannot gain or have Hunker tokens regardless of the Wounded/Not Wounded state of the other Unit.
The previous answer doesn’t take into consideration every possible exception created by interactions with other rules, just the rules and scenario presented.
